- Health insurance through Anthem Blue Cross/Blue Shield (PPO and HMO options)
Citizant offers 3 benefit plans to choose from in covering your health care needs. We offer a traditional HMO and two levels of PPO insurance plans (Key Care 15 and Key Care 25). The Key Care 25 PPO and the HMO Plan details are as follows:
- Citizant pays for 100% of the monthly premium cost for employees and 70% of the monthly premium cost for employees’ dependents
- Three-tier prescription plan
- Employee Assistance Program
- Anthem’s plans also include a vision discount program through BlueView Vision network
The Key Care 15 PPO is a premium benefit that employees may elect to buy up from the Key Care 25. The employee contributes towards the monthly premium cost for all coverages to include employee only and dependant coverage. - Vision insurance is provided through Vision Service Plan (VSP)
- Citizant pays 75% of the premiums and employees pay 25% of the premiums for all levels of coverage.
- VSP has a robust network of doctors
- Coverage includes exams, prescription lenses, frames (up to $120) and contact lens (up to $120)
- Also includes extra discounts and savings
- Dental insurance through Metlife
- Citizant pays 75% of the monthly premium cost for employees and dependents
- Utilizes Preferred Provider Network — best discounts when in-network
- Coverage levels vary by type: Preventative, Basic Restorative, Major Restorative, and Orthodontia coverage for children
- All white fillings
- Life insurance
- Citizant pays the premium for Basic Life Insurance at 100% of base salary (max. $50,000) through Unum-Provident for all full-time employees
- Employees have the option to purchase additional life insurance for themselves, spouse, and dependent children
- Life insurance benefit is portable
- Disability insurance
- Citizant pays the premium for a basic Short-Term and Long-Term Disability Insurance through Unum-Provident for all full-time employees
- Employees have the option to purchase additional disability insurance
- Flexible spending accounts (pre-tax benefit)
- Health care flexible spending account — $2,000 maximum annual contribution ($5,000 maximum beginning January 1, 2009)
- Dependent care - $5,000 maximum annual contribution

Many of Citizant’s top employees were referred to us by other employees. We encourage candidate referrals by rewarding employees when a referral is hired and works for us for 90 days. Most referral bonuses range from $2,000 (pre-tax) for a successful exempt position referral up to $10,000 for strategic, key business positions as determined by senior management.
